Account Activation Forms
All student accounts must be reactivated each semester, and be done by filling out the forms below. Accounts will not be activated unless the form is filled out completely and signed. One form must be filled out for each type of account. Completed forms can be submitted at Dana Hall 230 or Dana Hall 268. Once the forms are submitted, accounts may take one business day to become activated.
